Forms that need filled out and returned to school!
1) Handbook acknowledgement form. Due back to school by Tuesday 8/30/11.
2) The "Stuff you'll need" order form for 7th and 8th grade band. This order form includes places to order the concert shirts, book 2 (7th grade only), SmartMusic software and SmartMusic Microphones. Please turn this in to the band office in an envelope with the student's name, class period, and "order forms" on the outside. Orders not in an envelope will not be accepted. - This order form is for 7th and 8th grade only. Orders are due by Sept 2! Click here for the order form!
3) Smartmusic only order form - This order form is for 6th grade band members and anyone that needs to only order Smartmusic. Please note deadlines. Click here for the form!
4) Parent permission form for the SmartMusic Gradebook - Must be filled out by every 6th grade family. Due date is Thursday 11/17. Click here for the form!
