OHSAA Physical Forms will be available on-site or may be accessed below.
Only the OHSAA Physical Form dated March 2013-June 2014 will be accepted as valid.
Pay-to-Participate Fees
$200 per sport
$400 individual limit
$750 family limit (includes high school siblings)
This needs to be paid prior to the first day of competitionSpring Sports deadline is March 15, 2013
Make checks payable to Beavercreek City Schools
Put Athlete's first and last name in the memo line
